Fees for services are due and payable on the date and at the time of service. Fees for services include, but are not limited to the following: unpaid balances, deductibles, co-insurances, and co-payments. Payment for unpaid, outstanding balances is required BEFORE scheduling an appointment and before services can be rendered. Patient accounts must be current to obtain services. Non-payment of outstanding balances greater than 90 days will be turned over to a third-party collection agency.

The practice is out-of-network with all other insurances. If you choose to see a provider who is not covered under your insurance plan, you are considered a private-pay patient and are required to pay the full private-pay fee at the time of service.
For all in-network patients, insurance is verified prior to your visit to determine your benefits. Verification of benefits however, does not guarantee payment for services by your insurance company. Therefore, patients are responsible for all fees incurred, including co-payments, co-insurance, deductibles, and any other related fees. Current up-to-date insurance information is your responsibility. For out-of-network/private pay patients, the patient is responsible for obtaining any pre-authorization for visits and/or the verification necessary for obtaining reimbursement for your visit. Patients are responsible for all fees incurred. Current insurance information on-file is required for timely and accurate claims’ submission.
All charges are your responsibility whether your insurance company pays or does not pay. If your insurance carrier does not remit payment within ninety (90) days, the balance will be due in full from you. If any payment is made directly to you for services billed by Genesis Behavioral Health, you recognize the obligation to promptly remit payment to Genesis Behavioral Health.
